We are excited that you have chosen to register your child(ren) at Broadwater Creek Academy!  The registration timeline for  2024  is noted on our HOME PAGE.  Please contact the office directly for a hard copy of current registration forms.  Along with the registration form, we ask that you print out and/or complete the following:     

 

FOR YOUR RECORDS:

• Parent/Student Handbook  

  • Please read the handbook in full, as it highlights all of our school policies and procedures.

  • The handbook includes our School ID # and will direct you to the Lands’ End School Uniform Catalog.

• Parent's Guide to Regulated Childcare

 

FOR OUR RECORDS:

• Registration Form/Parent Agreement Form & Permission Slip (CALL OFFICE TO OBTAIN)

  • Please be as detailed as possible in your requests and return the forms with the necessary fees.  

  • Initial and sign where indicated on the Parent Agreement Form and Permission Slip.

  • You will receive a signed copy of this form or a confirmation letter/e-mail once your child has officially been registered in our program.

• Emergency Form

  • We will require this form to be updated annually in case of any changes/additions.

• Health Inventory

  • Part I – completed by parent

  • Part II/Lead Screening (updated 7/23)/Immunization Record – completed by pediatrician

• Topical Basic Care Product Application Authorization Form - MUST be completed for sunscreen/bug spray!!

• Registration Fee &/or Activity Fee

  • Noted on registration form 

• Please turn in ALL of the necessary paperwork/fees noted above at the same time. 

• The registration process will not be complete until we have received ALL things noted above.
